- Information submitted through our secure intake forms. New patients may complete secure, HIPAA-compliant intake forms hosted by Jotform prior to their appointment. Information submitted through these forms is subject to Jotform’s HIPAA-compliant handling and is used solely for treatment and administrative purposes related to your care.
- Patient Portal. Existing patients may access their health records through our third-party Patient Portal. Use of the portal is governed by that provider’s own privacy and security practices.
- Automatically collected information. Like most websites, we use Google Analytics to understand how visitors use our site. This may collect information such as your browser type, device type, pages visited, and general location (city/region level), typically through cookies or similar technologies. This information is aggregated and is not used to identify you personally.
